(This form is to be used to gather opinions while train riding, of train crews and yourself, as to the Network Operations Center’s performance. This form should only be completed to identify individual dispatcher performance excellence or deficiencies and/or to provide necessary information to the Mechanical Desk. This will eliminate any unnecessary paper work and provide employees with constructive feedback on their performance.
